Doctors Dont Recommend Alkaline Water

Heartburn (GERD): Why You Should Get Treated

Heartburn is caused by a flow of stomach acid backward into the esophagus, and drinking an alkaline solution may neutralize that fluid for a few minutes, according to Dr. Dellon. But it wont solve the underlying problem of why that acids there in the first place.

Its no different than taking a Tums or drinking a lot of milkwhich some people with reflux like to do because milk is also slightly alkaline, he says.

Drinking regular water may provide some relief, as well: It will raise the pH of your stomach, dilute the acid, and clear out the esophagusso theres lots of good reasons to drink water in general, and to stay hydrated, Dr. Dellon adds.

Patricia Raymond, MD, a physician in Norfolk, Virginia, and fellow of the American College of Gastroenterology, says her patients ask her about the alkaline diet and alkaline water as alternative treatments for reflux, but she doesnt recommend either.

I do support complementary medicine when we know that it can actually help, she explains. But the medical opinion is really lacking on alkaline water. Were not excited about it as a potential treatment.

Heartburn Acid Reflux And Gerd

The terms heartburn, acid reflux, and GERD are often used interchangeably. They actually have very different meanings.

Acid reflux is a common medical condition that can range in severity from mild to serious. Gastroesophageal reflux disease is the chronic, more severe form of acid reflux. Heartburn is a symptom of acid reflux and GERD.

Heartburn Symptoms And Complications

Gastroesophageal reflux disease , is a condition in which heartburn is a symptom. Stomach acid refluxes up into the esophagus and causes pain. This pain can be felt as a burning sensation behind the sternum or breastbone, either as a spasm or a sharp pain. Many times the pain of acid reflux can be mistaken for the pain of a heart attack.

The pain of acid reflux can remain in the lower chest or it can radiate to the back of the throat and be associated with waterbrash, a sour taste in the back of the throat. If there is acid reflux near the larynx in the throat, it may cause coughing episodes or hoarseness. Reflux over prolonged periods of time can be severe enough that acid wears away the enamel on teeth and causes decay.

Symptoms are often worsened after heavy meals, leaning forward, or lying flat. Those affected may often awaken from sleep with heartburn.

Complications

Heartburn is not without complications. If ignored, recurrent irritation and inflammation of the esophagus can lead to ulcers, which are small areas of tissue breakdown. These can cause serious bleeding.

As well, scarring and stricture are other significant complications of GERD. Changes in the type of cells lining the esophagus may result from acid reflux, causing a condition known as Barrett’s esophagus, which is associated with an increased risk of esophageal cancer.

Treating Heartburn Vs Indigestion

Both heartburn and indigestion may be treated with over-the-counter products, such as:

  • Antacids for mild, occasional heartburn symptoms. Antacids work by neutralizing stomach acid. They may be taken as soon as you experience symptoms, or before eating trigger foods to prevent them. Antacids arent meant to be taken every day, unless otherwise instructed by your doctor. Daily use can cause GI upset.
  • Proton pump inhibitors for long-term treatment. PPIs work by decreasing the amount of acid in your stomach, allowing your esophagus to heal.
  • Histamine-H2-receptor antagonists . These are also designed to decrease stomach acid, but they arent as strong as PPIs.

Ask your doctor before taking any herbal remedies for GI issues, as you may unintentionally make your heartburn or indigestion worse.

Occasional heartburn or indigestion is largely preventable. Prevention methods are the same for both conditions.

Learn What Causes Heartburn Symptoms And How To Control Them

That burning sensation in your chest and throat, often accompanied by a bitter taste in your mouth, is a common ailment known as heartburn. According to the American College of Gastroenterology, some 60 million Americans experience heartburn at least once a month, and about 15 million people have daily flareups. In this video, San Diego Health host Susan Taylor talks with Richard Onishi, MD, a family medicine physician with Scripps Clinic in Carmel Valley, about what causes heartburn and how to control it.

What is heartburn and what causes it?

Despite its name, heartburn has nothing to do with the heart. Heartburn is a form of indigestion, which is a broad term that includes upper abdominal pain often associated with food or other causes. Heartburn happens when acid in the stomach backs up, or refluxes, into the upper abdominal area and, sometimes, the esophagus and the back of the throat.

Your stomach is an acidic environment, and its protected by a layer of mucous, says Dr. Onishi. Unfortunately, the esophagus doesnt have that same protection, so if the acid refluxes up into the esophagus it causes a burning pain.

Along with burning in the chest and throat, heartburn symptoms may include regurgitation of stomach contents into the mouth, increased salivation, difficulty swallowing and a feeling of having something stuck in the throat. Some people may have a chronic cough.

Lifestyle Changes Help Treat Acid Reflux/gerd

  • Maintain a healthy weight
  • Avoid tight-fitting clothes
  • Avoid food and drinks that trigger heartburn
  • Watch portion sizes larger and higher-fat meals tend to stay in the stomach longer before moving to the small intestine, so the LES and esophagus are potentially exposed to stomach contents and acid for a longer period
  • Keep a heartburn record your symptoms, the time they occurred, what you ate and activities you engaged in before you experienced the discomfort
  • Dont lie down after a meal
  • Elevate your head when lying down

While some people experience acid reflux/GERD and heartburn relief by making changes to their lifestyle or by taking over-the-counter medications such as antacids, H2 blockers and PPIs, others may continue to experience symptoms. If four to eight weeks of twice-daily PPI therapy is unsuccessful, further investigation with endoscopy is recommended. There are some procedures that can be done to treat GERD.

When To See A Doctor About Heartburn Vs Indigestion

If your symptoms of heartburn and/or indigestion dont improve after a few weeks of home remedies and preventive measures, see your doctor.

Chronic heartburn or indigestion issues could be a sign of an underlying medical condition that needs treatment. To get to the root of chronic indigestion or heartburn issues, your doctor may order a few tests, including:

  • a physical exam of your abdomen
  • acid probe tests to measure when stomach acid reaches back into your esophagus
  • imaging tests, such as X-rays and endoscopies , to look at your esophagus and stomach
  • blood or stool tests to rule out bacterial infections that may be causing indigestion
  • pain in your abdomen that doesnt go away
  • frequent vomiting
  • blood in vomit or stools

Medical Treatments For Gerd

Treating Heartburn and GERD: Use Nexium, Prilosec, and other PPIs ...

Medications are available with and without a prescription for acid reflux and GERD.

Antacids: First-line treatments for acid reflux are usually antacids. These medicines act quickly to reduce the effect of stomach acid, which can relieve symptoms. Examples of these medicines are Tums and Rolaids.

If these medicines dont relieve acid reflux or a person has GERD, other treatments can include:

H2 blockers: H2 blockers are designed to reduce the amount of acid a persons stomach produces. Sometimes taking these medicines with antacids can help. Examples of these medicines include cimetidine and famotidine .

Proton pump inhibitors: These medications work longer than H2 blockers to reduce acid in the stomach. They can also help heal the stomach lining. Examples include:

Prokinetics: These are medications like metoclopramide . There is controversy as to whether these medications benefit people with GERD. Many new prokinetics have been removed from the market due to serious side effects .

If medications dont reduce a persons acid reflux symptoms, a doctor may recommend surgery to avoid further damage to the esophagus and stomach. One surgical approach is known as Nissen fundoplication. This involves wrapping a portion of your stomach around the esophagus to strengthen the LES.
