A total of 96 fresh fecal samples were collected from all four groups in the probiotic experiment at three time points and stored at 80°C. To characterize diversity, rarefaction was used to obtain 30,000 sequences per sample. Alpha diversity was represented using total species richness and evenness . Beta diversity was represented using UniFrac distances calculated from OTU counts. Statistical analyses were performed using SAS 9.3 and R 3.3.0 .

In a double-blind, placebo-controlled study versus placebo , the product was administered for 12 months to 40 IPPA patients at a dose of 1 sachet/day , with development of pouchitis in only 10% of cases compared with 40% in the placebo group .

    In a randomized trial versus no treatment enrolling 31 IPPA patients , only subjects receiving VSL#3 showed at 3, 6, and 12 months a significant reduction in the values of PDAI.

    A randomized, prospective study versus placebo employing a different probiotic mixture for 9 months in 43 patients recently submitted to restorative proctocolectomy found a significant reduction in the number of patients with pouchitis, as well as in PDAI scores and levels of fecal calprotectin in the probiotic group . The trial was enrolling both patients without pouchitis and subjects with active pouchitis.

    Recently, a very small, pilot study versus placebo from Japan has shown promising results in the primary prevention of pouchitis with Clostridium butyricum MIYARI over a follow-up period of 24 months.

    Arguably, there are two main approaches to selecting a strain to be probiotic. The first is to use a species normally abundant at a site, and simply replenish or boost the total count of that species in subjects whose microbiota has shifted to being dysbiotic.     One of the widely studied key features of LGG is its strong adhesive capacity, which has been documented in vitro but also in vivo in humans. LGG has been shown to be a very good mucus adhering Lactobacillus strain compared to related strains such as the dairy strain L. rhamnosus Lc705 and other probiotic strains such as L. johnsonii LJ1 and L. casei Shirota .1A). In human intervention studies, LGG was also reported to persist longer and in higher concentrations compared to closely related strains such as L. rhamnosus LC705 . Orally administered LGG can be recovered from the feces at least one week after administration in adults . Of note, the colonization capacity of LGG appears to be significantly better in newborns , which is related to reduced colonization resistance exerted by a less established microbiota of infants and which is probably a general feature for many probiotic strains. LGG can adhere to intestinal mucus and persist in the descending colon . Colonic biopsies even suggest that LGG colonization continues for longer than indicated by fecal recovery . LGG seems to show a preferential tissue tropism for the intestinal mucus layer, although these findings could be biased, as only limited studies have investigated the adherence and colonization at other body sites.
