It certainly appears probiotics can help manage IBS, but only particular strains and mixtures.

Just keep in mind that they should not be used as a sole therapy.

In other words, probiotics are most effective in combination with current conventional treatments such as a medications , and before or after a low FODMAP diet.

Also note that they can be a double-edged sword what works fantastic for some may only make symptoms worse in others. It likely depends on your existing gut bacteria environment.

For this reason you should be willing to experiment with different strains and different brands. Where one type doesnt help, maybe another will.

The most clinically proven and recommended brands among gastrointestinal specialists are VSL#3, Symprove and Alflorex/Align, in no specific order. But as always, speak with your doctor before trying anything new.

IBS affects our society in huge numbers – up to 20% of the UK population are sufferers, with women twice as likely to be affected as men. The most used diagnostic method for IBS is the Rome criteria, which classifies IBS into these subtypes:

IBS-D IBS-C IBS-A or IBS-M & IBS-U .

Although there is no clearly defined cause of IBS, many sufferers often report that symptoms are exacerbated by dietary factors such as wheat and dairy products, high FODMAPs foods, and lifestyle factors such as high-stress levels, as well as an imbalance in good and bad bacteria in the gut. It can be a good idea to keep a food and symptom diary for a few weeks to see if you have any dietary triggers.

Recent research may have found a link between IBS-D symptoms and a pathogenic bacterium called Brachyspira. This bacterium hides inside intestinal cells and is unaffected by antibiotic treatment. If more studies confirm this link between IBS symptoms and Brachyspira, researchers believe probiotics may become the future treatment22.

Irritable bowel syndrome is a chronic gut-brain disorder that can cause a variety of uncomfortable gastrointestinal symptoms including abdominal pain and diarrhea, constipation, or a mix of the two. IBS can reduce quality of life, often results in missed school or work, and can have a substantial economic impact.

    Advantages Of Inactive Probiotics

    Previously, general consensus held that only active, living bacteria may have beneficial effects. But these results suggest that heat-inactivated Bifidobacterium can play a significant role in relieving symptoms of IBS, a syndrome with typically limited options for relief.

    This is important because inactive probiotics have several potential advantages over active probiotics. For example, they are more likely to be stable, particularly if exposed to excessive heat. Inactive probiotics are also easier to standardize than active probiotics. Whether other strains of heat-inactivated probiotics will also improve IBS symptoms remains unknown.

    Trying Ibgard For My Ibs

    I was in the grocery store and decided to spend a couple of extra bucks on the Align probiotic and saw IBgard sitting on the shelf nearby. I have heard a lot of folks on the site talk about the relief they have received from this product and peppermint oil in general. I have included raw peppermint in my diet at times with good results. Nevertheless, I threw caution to the wind and purchased both things, along with some bone broth.

    I have to say, over the last month, I feel much better. It looks like I may be able to avoid surgery for the gallstones and my digestive system feels like is really in a much healthier state. I can feel it. I know that most of us with IBS are very in touch with our tummies and the physiological systems that surround it. I think we know when our digestive systems are doing well and when they could use a little work. I dont know that this will be a permanent state of calm , but for the moment I am quite pleased with my choice and I thought I would share.

    Lactobacillus rhamnosus includes a wide variety of probiotic strains. Other strains such as Lactobacillus rhamnosus Lc35 have been shown to produce regular bowel movements. This aids those who suffer from constipation. This particular strain has been shown to be effective by increasing mucous secretions and converting bound bile salts into free bile salts. Then, these free bile salts act to pull water into the colon. Most importantly, in combination with the increased mucous secretions, it softens stool and aids in regular bowel movements.

    Recent Study Finds Inactive Probiotics Relieve Ibs Symptoms

    More recently, Bifidobacterium bifidum MIMBb75 has been shown to improve symptoms of IBS, even in its inactivated form. For the eight-week, double-blind, placebo-controlled clinical trial published in Lancet Gastroenterology, researchers studied whether the heat-inactivated form of Bifidobacterium bifidum MIMBb75 could alleviate IBS symptoms.

    Overall, 443 patients were randomized to receive heat-inactivated Bifidobacterium bifidum MIMBb75 or placebo once daily. A total of 377 patients completed the trial. The primary endpoint was defined as a 30% or greater improvement of abdominal pain and at least somewhat relieved overall IBS symptoms for four or more weeks of the eight-week study duration. Significantly more patients receiving the Bifidobacterium probiotic met the primary endpoint compared to patients receiving placebo . In addition, a significantly greater percentage of patients receiving the probiotic also reported adequate relief of symptoms compared to placebo. Finally, individual symptoms including bloating, bowel movement satisfaction, and quality of life were also significantly improved with Bifidobacterium bifidum MIMBb75 compared to placebo.

    Side Effects And Safety

    Probiotic products may worsen or cause symptoms in some people, whether they have IBS or not. Temporary gas and bloating are sometimes reported in the first few weeks people try taking a probiotic. Depending on the ingredients, probiotics may cause symptoms, side effects, or reactions in people with other medical conditions or food allergies.

    While there’s no guarantee they will help, most people don’t experience any serious side effects from taking probiotics. However, if you’re concerned about the safety of a product, you’ll need to check with the manufacturer.

    Companies usually test their products for safety and effectiveness before they hit store shelves, but probiotic supplements aren’t consistently regulated in the United States.

    A lack of regulation means that probiotic products don’t need to be approved by the FDA before they can be sold, while others do .

    Probiotic supplements may not cause side effects for most healthy people, but that doesn’t mean they’re suitable for everyone. If you have certain health conditions, food allergies, or a weakened immune system, your healthcare provider might advise against taking probiotics.

    What About Kijimea And Saccharomyces Boulardii

    Kijimea is a popular IBS probiotic pill in the US.

    It contains a strain of bacteria called Bifidobacterium bifidum MIMB75 and is the only product with this strain.

    Theres been one clinical trial using this strain, which included 122 patients. After 4 weeks, 47% of those taking B. bifidum MIMB75 reported significant improvements in pain, bloating, urgency and digestive disorders . Whereas 11% of patients taking the placebo pill reported benefits .

    So Kijimea certainly shows some potential, but the science supporting it is thin. The popularity likely comes from the fact it is easily and has great user reviews.

    Should Probiotics Be Taken On A Low Fodmap Diet

    The low FODMAP diet is one of the proven methods for treating IBS.

    It involves first eliminating most FODMAPs from the diet for a 3-8 week period so that you are virtually symptom-free. Then you begin to systematically reintroduce them in order to identify the culprits, followed by a modified diet long term.

    For this to work, its important that other possible triggers of digestive stress are minimized. That includes caffeine, spicy foods, fatty foods, alcohol, and fiber supplements among other things. Removing these ensures that if symptoms do occur, you will be able to trace it back to an exact FODMAP group.

    While probiotics can be highly beneficial, some people experience a worsening of symptoms. Given this risk, its typically not recommended to start a new probiotic supplement during any phase of the low FODMAP diet.

    The exception is if you already use a probiotic supplement before starting a low FODMAP diet. In this case you you can continue to do so throughout.

    Summary: Some people may experience even worse digestive symptoms when starting a new probiotic supplement. This will compromise the results of a low FODMAP diet, so its not recommended to do them at the same time.
