Ondansetron is an anti-nausea medication used to prevent nausea and vomiting that results from cancer chemotherapy. Chemotherapy agents cause increased secretion of serotonin, which stimulates serotonin receptors in the brain, resulting in nausea and vomiting. Ondansetron works by selectively blocking serotonin receptors, thereby reducing the effect of increased serotonin due to chemotherapy. It is also prescribed for prevention of nausea and vomiting after surgery.

Metoclopramide is a prokinetic drug that stimulates the muscles of the gastrointestinal tract including the muscles of the lower esophageal sphincter, stomach, and small intestine by interacting with receptors for acetylcholine and dopamine on gastrointestinal muscles and nerves. It is used in adults to relieve heartburn symptoms with gastroesophageal reflux when certain other treatments do not work and to relieve the symptoms of slow stomach emptying in people with diabetes.

Why Treat Heartburn And Acid Reflux During Pregnancy

Heartburn and acid reflux are traditionally considered innocuous because they are common in pregnancy and are usually self-limiting. However, a recent study suggests that GERD is associated with an increase in the severity of nausea and vomiting of pregnancy , which can have serious negative effects on a womans quality of life. In a prospective cohort study conducted by the Motherisk Program, 194 pregnant women with NVP and heartburn or acid reflux were compared with 188 pregnant women with NVP who did not have heartburn or acid reflux. The 2 groups were assessed for severity of NVP with the pregnancy-unique quantification of emesis and nausea score, which is a validated scoring tool based on frequency and duration of nausea, vomiting, and retching in the past 24 hours, with a high score indicating more severe NVP. Quality of life was measured by the well-being score, with 0 being the worst and 10 being the best. Forty-eight percent of women in the control group had severe NVP according to the PUQE, compared with 75% in the group with heartburn and acid reflux. The mean well-being score in the control group was significantly higher than that in the group with heartburn and acid reflux .

How Should I Take Zofran

Take Zofran exactly as prescribed by your doctor. Follow all directions on your prescription label. Do not take this medicine in larger or smaller amounts or for longer than recommended.

Zofran can be taken with or without food.

The first dose is usually taken before the start of your surgery, chemotherapy, or radiation treatment. Follow your doctor’s dosing instructions very carefully.

Take the regular tablet with a full glass of water.

To take the orally disintegrating tablet :

  • Keep the tablet in its blister pack until you are ready to take it. Open the package and peel back the foil. Do not push a tablet through the foil or you may damage the tablet.

  • Use dry hands to remove the tablet and place it in your mouth.

  • Do not swallow the tablet whole. Allow it to dissolve in your mouth without chewing.

  • Swallow several times as the tablet dissolves.

Patients with irritable bowel syndrome with diarrhea suffer not only the pain and discomfort of frequent loose stools, but fear of incontinence. For many patients, that results in panic and anxiety.

In an effort to address those concerns, a team at Nottingham Diseases Centre, University Hospital, Nottingham, UK, conducted a study of ondansetron vs. placebo in a group of 120 IBS-D patients. The group taking ondansetron got significant relief from their debilitating symptoms, Klara Garsed and colleagues reported in Gut .

Compared with placebo, ondansetron significantly improved stool consistency, patients experienced fewer days with urgency, less frequent defecation, and less bloating. Pain associated with IBS-D did not change significantly.

Overall, 65% of patients reported symptom relief with ondansetron, while only 14% getting the placebo reported relief.

In the crossover study, participants received 5 weeks of oral placebo treatment and 5 weeks of ondansetron 4 mg tablets with a 2-3 week washout period in between each treatment period.

Loperamide was allowed as a rescue medication but needed to be discontinued for the final 2 weeks of the period. Patients kept track of their symptoms with daily diaries.

In the trial, all patients had IBS-D as defined by the Rome III criteria. After a 2- to 3-week washout period in which patients did not take medication for IBS, investigators randomized 120 patients to each treatment group in an approximately 1:1 ratio. Patients then received 5 weeks of treatment with ondansetron or placebo and recorded bowel symptoms in a diary. Over the first 3 weeks, patients were instructed to increase the dose slowly to a maximum total daily dose of 24 mg . Despite these instructions, the median dose of ondansetron used in the trial was 4 mg daily.

Ondansetron works by blocking a subtype of serotonin receptors associated with nausea . Blocking this serotonin receptor may also reduce the serotonergic signaling that induces symptoms of loose stool and bowel urgency in patients with IBS.

Tell your doctor about all medicines you use, and those you start or stop using during your treatment with ondansetron.

Receiving ondansetron while you are using certain other medicines can cause high levels of serotonin to build up in your body, a condition called “serotonin syndrome,” which can be fatal. Tell your doctor if you also use:

  • medicine to treat depression
  • medicine to treat a psychiatric disorder
  • a narcotic medication or
  • medicine to prevent nausea and vomiting.

This list is not complete and many other drugs can interact with ondansetron. This includes prescription and over-the-counter medicines, vitamins, and herbal products. Give a list of all your medicines to any healthcare provider who treats you.

Causes And Risk Factors

There are several risk factors that are considered to play a role in the conditions cause, such as vagus-nerve damage. The vagus nerve is the longest cranial nerve in the body and is responsible for many functions. It is especially essential for proper operation of the digestive tract. If the vagus nerve is damaged, transfer of food from the abdomen to the small intestine is reduced because the muscles will not operate properly.4

Type 1 and type 2 diabetes are known to damage the vagus nerve. Some autoimmune diseases and virus infections are also believed to have a negative impact on the vagus nerve. In certain cases, the vagus nerve stops working properly due to drinking excessive alcohol. Surgical complications could also affect the vagus nerve.4

Other factors that can increase the risk of gastroparesis include abdominal or esophageal surgery, infection , certain medications that slow the rate of stomach emptying , nervous system diseases and hypothyroidism.4 Complications resulting from gastroparesis are shown in TABLE 1.

Sometimes, treating the cause may stop the problem. If diabetes is causing gastroparesis, patients must control their blood glucose levels. Acute hyperglycemia may impair gastric motor function as well as inhibit the action of prokinetic drugs, such as erythromycin. In patients with type 1 diabetes, gastroparesis can be an indication for insulin-pump therapy.5
