Does anyone have any idea what I can do to help with my eating? Nearly every time I eat a meal I struggle with gas and burps along with the usual IBS discomforts which I have previously documented on this forum. I am following the Fodmap foods, do not have gluten, eat plenty of fruit and vegetables, drink plenty of water and take regular exercise. I have small mouthfuls of food, chew thoroughly and take rests between each mouthful. For example tonights meal was a chicken breast and jacket potato and I struggled to eat it because of the burping.

Fewer side effects than Proton Pump Inhibitors Ranitidine is a H2 blocker with the medical proffesion these have gone out of fashion since the new and more expensive Proton Pump Inhibitors came on the scene. I have Barrett’s Esophagus caused by acid reflux. For me proton pump inhibitors don’t cut it because of side effects, they cause dry eyes, flare ups of my colitis and worst of all erectile dysfunction. My wife also takes a Proton Pump Inhibitor and suffers from dry eyes and vision blurring but Ranitidine just dosen’t work for her. On changing to Zantac Initially I was precribed 150mg Zantac twice per day but found it only stopped acid production for 8 to 10 hours leaving a 2 to 4 hour gaps when heartburn became a problem. I persuaded my doctor to try putting me on 75mg three times per day and this worked. More recently I have tried cider apple vinegar therapy once a day to reduce acid production to my great surprise it works and I am now back down to 75mg Zantac twice per day. Strangely Cider Apple Vinegar and also Red Grapefruit Juice appear to aid healing of uclers and acid burn damage.

A meta-analysis of RCTs concluded that antispasmodics were superior to placebo in the treatment of IBS , with an NNT of 5.212 Overall, there was an improvement of abdominal pain and global symptoms in the pooled analyses however, the quality of most trials was low, results were heterogeneous, and publication bias could not be excluded. Moreover, only anticholinergic antispasmodic agents are available in the USA. Non-anticholinergic antispasmodics that appear to be efficacious include otilonium and certain selective calcium channel blockers .213

Peppermint oil is also efficacious for global symptoms and abdominal pain, and is usually well tolerated the NNT is 3 .212 A novel formulation, designed for sustained release in the small intestine, is available for use in the USA.214 The usual dose of peppermint oil is 180 to 200 mg 3 times a day 30 minutes before meals AEs include heartburn, perianal burning, and rarely interstitial nephritis.

Abdominal pain or discomfort should be present at least 3 days per month for 3 months and should be associated with two or more of the following: improvement with defecation, onset associated with a change in stool frequency and onset associated with a change in stool form . The American College of Gastroenterology guidelines emphasize a clinically oriented approach and define IBS as lower abdominal pain or discomfort with disordered defecation .

Why Heartburn And Gas May Occur At The Same Time

Gas and heartburn are not always connected directly. However, it is possible for the two to exist together and make each other worse. It is also possible for a person to manage them in a similar way.

One common cause of heartburn and gas is eating too much too quickly. Eating large amounts of food in one sitting can trigger heartburn, while eating quickly may cause a person to swallow more air, resulting in gas.

Drinking fizzy alcoholic drinks, such as beer, can also cause both conditions to co-occur.

Relieving gas through belching can cause heartburn to worsen. The release of air when someone passes gas either during or immediately after a meal can cause more acid to rise in the throat. If this occurs, a person may experience heartburn.

People can treat heartburn directly with a couple of different types of medication that are available on prescription or over the counter .

What If I Forget To Take It

If you forget to take a dose, take it as soon as you remember. If you do not remember until it’s nearly time for the next dose, skip the missed dose and take the next one at the usual time.

Never take 2 doses at the same time. Never take an extra dose to make up for a forgotten one.

If you often forget doses, it may help to set an alarm to remind you. You could also ask your pharmacist for advice on other ways to remember to take your medicines.

What Is Ranitidine And Which Products Are Affected

Ranitidine is available both over the counter and by prescription. It belongs to the class of drugs known as H2 blockers. OTC ranitidine is commonly used to relieve and prevent heartburn. Cautions With Other Medicines

Some medicines and Buscopan can affect each other and increase the chances of you having side effects.

Do not take Buscopan at the same time as some motion sickness medicines, such as domperidone and metoclopramide.

This is because they can cancel each other out and stop each other from working. Metoclopramide and domperidone increase the movement of the gut, whereas Buscopan reduces it.

Some medicines increase the risk of side effects if you take them at the same time as Buscopan, including:

  • amantadine
  • quinidine or disopyramide
  • some inhaled asthma medicines, including salbutamol, ipratropium and tiotropium

Do not take Buscopan together with other irritable bowel syndrome remedies, as they work in the same way. You’re unlikely to get extra relief for your symptoms, and it may cause more side effects. Take only one IBS remedy, unless your doctor tells you to take more than one.

Since mid-2019 there has been an investigation of a contaminant known as N-Nitrosodimethylamine found in ranitidine medications. NDMA is thought to be a human carcinogen something that could cause cancer. Very low levels of NDMA are commonly found in foods and in water. They can be eaten and tolerated in small amounts. At these low levels, NDMA is not thought to lead to an increased risk of cancer. But at higher levels of long-term exposure, it may increase the risk of cancer in humans.1

This impurity, present in some products, has been found to increase over time and when stored at higher than room temperatures. This could potentially cause consumer exposure to higher, unacceptable levels of NDMA.1-2

How To Cope With Side Effects Of Buscopan

What to do about:

  • dry mouth â chew sugar-free gum or suck sugar-free sweets
  • constipation â get more fibre into your diet, such as fresh fruit, vegetables and cereals, and drink plenty of water. Try to exercise more regularly, for example, by going for a daily walk or run. If this does not help, talk to your pharmacist or doctor.
  • blurred vision â do not drive or ride a bike until you can see clearly again. Do not take your next dose of Buscopan if your vision is still blurry. Speak to your doctor or pharmacist if your vision has not returned to normal a day after taking your last dose.
  • fast heart rate â if this happens regularly after taking Buscopan, try to take your medicine at a time when you can sit or lie down when the symptoms are worst. It may also help to cut down on alcohol, smoking, caffeine and big meals, as these can make the problem worse. If you’re still having symptoms after a week, speak to your doctor. You may need to change to a different type of medicine.

I’ve been taking ranitidine 300 mg a day for ibs and gall bladder disease for a week and a half. The symptoms of the ibs and gall bladder disease improved. However I have experienced severe side effects. Extreme irritability, constant headaches, extreme insomnia, horrible, horrible nightmares when I do manage to sleep. An erection? Not since I started taking this medication. I feel a little mentally unstable since I started this medication. Obviously for me the side effects far outweigh any relief I have experienced using this drug.

Hi, I am new to this! I have recently been diagnosed with IBS after 2 1/2 years of chronic pelvic pain. Have got large adhesion on my omentum . I am due to see an IBS specialist nurse next month. I am trying to eat gluten/wheat/lactose free. I would like to know if anyone else feels constantly tired and like functioning on autopilot? I am so tired i can easily go to bed at 7pm and altho i dont go to sleep immediately….i feel better for being laid down. Is this normal? I work part time and have 3 kids and i am in bed before they are! Thank you x

    This is completely normal I have suffered with I b s for nearly 16 years I also get days where u feel totally drained I had 2 operations to remove my adhesions but think they may be back but trying new medication to help with my I b s symptoms and constipation so fingers crossed that helps do you take anything for your I b s ?

    Hiya, thanks for the reply! I take Citalopram …..was on that though before all this started….i take co-codamol for pain relief but i also have liquid morphine if pain is severe, i have buscopan for the cramps and take ppi cover in the form of ranitidine. I just hate feeling so tired….and not even yawny tired…its a “im very drained” tired. I havent been so good with my food this past few days, eating things i know i shouldnt but the tiredness is there no matter what i do or dont eat? Xx

Some Things Help Others Don’t

Although there are all sorts of tips and recommendations out there when it comes to IBS treatment, there’s a lack of good research in this area. Generally speaking, it’s fair to say that there’s no treatment that is guaranteed to relieve the symptoms. But research has shown that some treatments can at least help in some people or in the short term. These include the following:

  • Dietary supplements containing peppermint oil or probiotics
  • Certain anti-cramping medications, antidepressants and drugs to relieve constipation
  • Psychological treatments such as cognitive behavioral therapy and hypnosis

But research has also shown that these treatments particularly the medications can cause side effects.

Studies on the effects of have found that it doesn’t help in IBS.

Many other recommended treatments haven’t been studied properly so it isn’t clear how well they work. These include reflexology massage, enemas and changing what you eat for instance, more fiber or certain diets.
