What Are Ondansetron And Metoclopramide
Ondansetron is an anti-nausea medication used to prevent nausea and vomiting that results from cancer chemotherapy. Chemotherapy agents cause increased secretion of serotonin, which stimulates serotonin receptors in the brain, resulting in nausea and vomiting. Ondansetron works by selectively blocking serotonin receptors, thereby reducing the effect of increased serotonin due to chemotherapy. It is also prescribed for prevention of nausea and vomiting after surgery.
Metoclopramide is a “prokinetic” drug that stimulates the muscles of the gastrointestinal tract including the muscles of the lower esophageal sphincter, stomach, and small intestine by interacting with receptors for acetylcholine and dopamine on gastrointestinal muscles and nerves. It is used in adults to relieve heartburn symptoms with gastroesophageal reflux when certain other treatments do not work and to relieve the symptoms of slow stomach emptying in people with diabetes.
Why Treat Heartburn And Acid Reflux During Pregnancy
Heartburn and acid reflux are traditionally considered innocuous because they are common in pregnancy and are usually self-limiting. However, a recent study suggests that GERD is associated with an increase in the severity of nausea and vomiting of pregnancy , which can have serious negative effects on a womans quality of life. In a prospective cohort study conducted by the Motherisk Program, 194 pregnant women with NVP and heartburn or acid reflux were compared with 188 pregnant women with NVP who did not have heartburn or acid reflux. The 2 groups were assessed for severity of NVP with the pregnancy-unique quantification of emesis and nausea score, which is a validated scoring tool based on frequency and duration of nausea, vomiting, and retching in the past 24 hours, with a high score indicating more severe NVP. Quality of life was measured by the well-being score, with 0 being the worst and 10 being the best. Forty-eight percent of women in the control group had severe NVP according to the PUQE, compared with 75% in the group with heartburn and acid reflux. The mean well-being score in the control group was significantly higher than that in the group with heartburn and acid reflux .

What Happened To Mylanta
For many years, especially in the 1990s, Mylanta was a popular product used to alleviate the symptoms of heartburn. However, in 2010, the over-the-counter antacid was voluntarily recalled due to traces of alcohol being found in the product.
According to the manufacturer, Johnson & Johnson, the product was recalled so it could be relabelled with better accuracy, not because of any risk to alcohol absorption or adverse effects.
In 2016 Mylanta was reintroduced to the market and deemed safe to use as an antacid.

Doctors call it gastroesophageal reflux disease, or GERD. Millions of men call it heartburn, and many others have coughing, wheezing, or hoarseness without realizing that GERD is to blame. By any name, GERD is common, bothersome, and sometimes serious. It is also expensive, draining the American economy of more than $9 billion a year. But once you know you have GERD, you can control it and prevent complications.
How Should I Give It
Ondansetron may be given by mouth as a liquid, pill, or tablet or in a vein . It may be given in one of these ways:
- as needed
- at regular times
- continuously through an IV
Your child should be awake and alert when taking any medicine by mouth. Follow the checked instructions below:
___ If using the liquid form, draw up the correct amount in the medicine dropper or oral syringe. Give a small squirt of the medicine inside the cheek. To avoid choking, let your child swallow each squirt before giving more.
___ For babies, you may want to mix the medicine with a small amount of formula or breast milk and give it with a bottle nipple before feeding. Do not add the medicine to a whole bottle because if your baby does not finish it, you will not know how much of the medicine was taken.
___ For older children who cannot swallow pills, a special tablet called Zofran ODT is available. Place it under the tongue and it will dissolve.

Getting The Most From Your Treatment
- Ondansetron should start to work within an hour or so. If you are taking tablets or liquid medicine and you are sick within one hour of taking a dose, you should take the same dose again. This does not apply, however, if you are using suppositories or if you have been given an injection, as being sick will not prevent these from being absorbed by your body.
- Even if you do not feel like eating or drinking, try to sip water regularly to prevent you from becoming lacking in fluids . Also, rather than trying to eat three main meals a day, try eating small, simple but nourishing snacks, every few hours.

Gastroparesis is a chronic disorder that affects a significant subset of the population. Ordinarily, strong muscular contractions move food through the digestive tract. In gastroparesis, this mechanism is disrupted, and undigested food stays in the abdomen for a long time and makes a person feel nauseous with the urge to vomit. Gastroparesis can also cause a lack of appetite, which may lead to malnutrition, and patients who are not eating can expect discomfort, bloating, and heartburn.1
The pathophysiology behind gastroparesis is varied and depends on disease etiology. Vagal and/or autonomic neuropathy play an important role in the development of diabetic gastroparesis, and it is estimated to occur in up to 20% to 40% of patients with diabetes. Gastroparesis can cause problems with blood sugar levels and nutrition. Sometimes, it is a complication of diabetes, and some people may develop gastroparesis after surgery. Although there is no cure for gastroparesis, changes to the diet, along with medication, can offer some relief.1,2

Typical Dosing For Zofran
Adults:

Chemotherapy that causes severe nausea and vomiting: a single 24 mg dose taken 30 minutes before the start of a single-day highly vomit-inducing chemotherapy.

Chemotherapy that causes moderate nausea and vomiting: 8 mg dose taken 30 minutes before the start of chemotherapy, with another 8 mg dose taken 8 hours after the first dose. Take 8 mg twice a day for 1 to 2 days after completing chemotherapy.

Radiation therapy: 8 mg dose taken 1 to 2 hours before radiation therapy. Depending on the type of radiation therapy and location of radiation therapy, 8mg dose can be taken every 8 hours after the first dose for each day radiation therapy is given or for 1 to 2 days after completing radiation therapy.

After surgery : 16 mg dose taken 1 hour before starting anesthesia.
Children :

How To Prevent Medication
Not everyone will become constipated when using these medications. However, you should be aware of the possible side effects and what you can do to prevent constipation.
In general, you want to make sure you are well hydrated with water, not coffee or carbonated beverages, says Dr. Curtin. You also want to have a healthy amount of fiber in your diet and try to limit processed foods. It is worth asking your doctor if there are alternative agents. Calcium channel blockers, used for high blood pressure, such as diltiazem, for example, are notorious for causing constipation. If there is another class of medication that can treat the underlying health problem, you should at least be given the option.

What Is The Dosage Of Ondansetron Vs Metoclopramide
Ondansetron
Dosing for adults:
- Highly nauseating chemotherapy: 24 mg orally dissolved on tongue 30 minutes prior to start of a single-day chemotherapy.
- Moderately nauseating chemotherapy: Take 8 mg tablet 30 minutes prior to chemotherapy and repeat in 8 hours, then 8 mg every 12 hours for 1 to 2 days after chemotherapy.
- Radiation-induced nausea and vomiting: Take 8 mg orally 1 to 2 hours prior to radiation and every 8 hours after first dose, as needed.
- Post-surgery nausea and vomiting: 16 mg orally 1 hour before anesthesia.
Dosing for children:
- Moderately nauseating chemotherapy : 8 mg orally prior to chemotherapy and repeat in 8 hours, then 8 mg every 12 hours for 1 to 2 days after chemotherapy.
- Moderately nauseating chemotherapy : 4 mg orally 30 minutes prior to chemotherapy and repeat in 4 and 8 hours after the first dose, then every 8 hours for 1 to 2 days after chemotherapy.
- Not recommended for children under 4 years old.
Metoclopramide
The usual dose of metoclopramide for treating GERD is 10-15 mg four times daily, 30 minutes before each meal.
Gastroparesis is treated with 10 mg administered orally four times daily, 30 minutes before each meal and at bedtime.
