Surgery For A Hiatal Hernia

Can Hernia mesh cause problems years later? Signs, Symptoms, Treatment – Dr. Nanda Rajaneesh

Surgical treatment is sometimes required to repair larger hiatal hernias and treat heartburn symptoms. Surgery for Hiatal hernias is used if reflux symptoms can not be successfully controlled with medication or if you have a type of hernia called a giant esophageal hernia, also known as intrathoracic stomach.

What Are The Complications Of Inguinal Hernias

Inguinal hernias may become stuck, or incarcerated, meaning the contents of the hernia that bulge through the abdominal wall cannot be massaged back inside the abdominal wall.

If a hernia becomes stuck outside the abdominal wall, it may become strangulated, meaning the blood flow to the hernia is cut off. Lack of blood flow can cause the death of tissues inside the hernia.

If a hernia that contains part of the small intestine becomes stuck and strangulated, this can lead to intestinal obstruction and death of the strangulated part of the intestine.

Symptoms Of An Epigastric Hernia

An epigastric hernia usually causes a bump to occur in the area below your sternum, or breastbone, and above your belly button. This bump is caused by a mass of fat that has pushed through the hernia.

The raised area may be visible all the time or only when you cough, sneeze, or laugh. This bump, or mass, can grow and become larger in some cases. You can have more than one epigastric hernia at a time.

An epigastric hernia can also cause tenderness and pain in the epigastric region. However, its common for an epigastric hernia not to show any symptoms.

Other types of hernia that can affect the tummy include:

  • incisional hernias where tissue pokes through a surgical wound in your tummy that has not fully healed
  • epigastric hernias where fatty tissue pokes through your tummy, between your belly button and the lower part of your breastbone
  • spigelian hernias where part of your bowel pokes through your tummy at the side of your abdominal muscle, below your belly button
  • diaphragmatic hernias where organs in your tummy move into your chest through an opening in the diaphragm this can also affect babies if their diaphragm does not develop properly in the womb
  • muscle hernias where part of a muscle pokes through your tummy they also occur in leg muscles as the result of a sports injury

Hiatus Hernia And Heartburn

A hiatus hernia itself rarely has any noticeable symptoms. However, it can cause a problem called gastro-oesophageal reflux disease .

GORD is where stomach acid leaks into the oesophagus . It can occur if a hiatus hernia prevents the valve at the bottom of the oesophagus from working properly.

Your oesophagus can become severely irritated, because its not protected against stomach acid. This can cause symptoms such as heartburn, chest pain, an unpleasant sour taste in your mouth, and swallowing problems .

You should see your GP if you have frequent and severe symptoms of GORD.

Types Of Hiatal Hernia

  • The sliding hiatal hernia: Occurs when part of your stomach slides up to the chest through the esophageal opening in the diaphragm. This type typically presents with GERD . However, many cases have no symptoms at all .
  • The paraesophageal hiatal hernia: Another less common type, in which part of the fundus of the stomach herniates through the diaphragm into the chest. Vague and atypical symptoms characterize this type.

Emergency surgery is needed for a hiatal hernia if your stomach is being squeezed so tightly that its blood supply is cut off.

Surgery may also be needed if you have severe GERD that isnt responding well to lifestyle measures and medication.

To surgically repair a hiatal hernia, your doctor will pull the entire stomach back down into your abdomen and make the opening in your diaphragm smaller.

If needed, your surgeon may also repair your esophageal sphincter the muscle that normally prevents your stomach contents from flowing back into your esophagus.

This surgery is usually done laparoscopically involving several small incisions and using a flexible tube containing a light and camera to view the inside of your abdomen and chest.

Causes And Risk Factors Of A Hiatal Hernia

Can Hernia lead to back pain & flatulence? – Dr. Nanda Rajaneesh

Hiatal hernias can happen because of weakening in the muscle tissue of your diaphragm, which allows your stomach to push up through the opening for your esophagus.

The specific causes of a weakened diaphragm arent always clear, but the following factors may play a role:

  • Age-related changes to your diaphragm
  • Injury from trauma or surgery
  • Being born with a large hiatus

A weakened diaphragm usually isnt enough to cause a hiatal hernia you also need increased pressure in your abdomen to push the stomach up through the diaphragm.

Increased abdominal pressure can come from the following factors:

  • Strenuous exercise
  • Lifting heavy objects

Hiatal hernias can occur at any age and affect both sexes. The following factors, though, make it more likely that youll experience one:

  • Being 50 or older

How To Tell If You Have A Hernia

Wondering if the aching in your belly or groin could be a hernia? Hernias are common in both men and women, but symptoms vary. Groin hernias are much less common in women. Hernias may cause some form of discomfort and dont go away on their own, says Gina Adrales, M.D., M.P.H., director of the Division of Minimally Invasive Surgery at Johns Hopkins Medicine.

The good news? Nearly all hernias can be surgically repaired. Adrales explains how to recognize hernia signs in men and women, the most common types, and what surgery and recovery are like.

How Is A Hiatal Hernia Treated

Most hiatal hernias do not cause problems and rarely need treatment. However, since some patients with a hiatal hernia have symptoms of GERD, treatment starts with methods used to manage GERD. These include making such lifestyle changes as:

  • Maintaining a weight that’s healthy for you.
  • Avoiding certain acidic foodssuch as tomato sauce and citrus fruits or juicesthat can irritate the esophageal lining.
  • Limiting fried and fatty foods, foods or drinks containing caffeine , peppermint, carbonated beverages, alcoholic beverages, ketchup and mustard, and vinegar.
  • Eating meals at least three to four hours before lying down, and avoiding bedtime snacks.
  • Keeping your head six inches higher than the rest of your body when lying on your back. Raising the level of your head helps gravity keep your stomachs contents in the stomach. Raising the head of your bed by angling your mattress works bestpiling your pillows doesnt work as well because it makes you crunch your middle instead of simply angling your body upwards.
  • Quitting smoking.
  • Not wearing a tight belt or tight clothing that can increase the pressure on the abdomen such as control top hosiery and body shapers.
  • Taking medications after eating to reduce acid in the stomach. These over-the-counter medications include antacids, Gaviscon®, or H-blockers .

Do We Need A New Way To Treat Sibo

While SIBO is a relatively new diagnosis, understanding of the process of methanogenesis has existed for decades.

In fact, methanogenesis has already been studied exhaustively in the agricultural world. This is because the production of excessive methane gas by cattle can actually impact the atmosphere.

Because it means there is a time-tested and science-backed way to treat excess methane in the gut caused by SIBO other than just drastically modifying the diet.

This idea was first explored by Dr. Kenneth Brown a board-certified gastroenterologist and clinical researcher.

Instead of trying to starve the bacteria out or obliterate them with antibiotics Dr. Brown wanted to interrupt the process of methanogenesis.

Does It Matter If Youre A Methane Producer

While it is normal and healthy to have a diverse bacteria in your colon including the presence of methanogens if youre primarily a methane-producer, it can help to explain why you struggle with bloating.

    • Methane producers suffer more frequently from bloating than non-producers
    • Methane producers also suffer more frequently with abdominal pain and gas
    • Methane acts as a paralytic to slow down gastrointestinal transit time

And if youre struggling with constipation or maintaining a healthy weight?

    • In those reporting constipation, higher amounts of M. smithii have been found
    • The amount of M. smithii inversely correlates to stool frequency
    • Research studies have proposed that the presence of M. smithii also increases caloric absorption because of its effect on metabolism
    • Other studies have suggested that methane-producers have reduced postprandial serotonin levels, which can also affect intestinal transit

Q: What Are The Signs Of A Hernia Emergency

A: When most hernias start, the internal tissue that pushes through the muscle gap is usually fat. But its also possible for part of the bowel to protrude through the opening. The risk of this is very low, but when it happens, it needs emergency repair. Seek immediate medical attention if you have:

    • A painful bulge that doesnt reduce in size when you lay down and rest

    Complications Of Hiatal Hernia

What can I expect after hernia surgery? How long is my recovery?

In the majority of cases, a hiatal hernia will not lead to other health problems. However, in some instances, it can lead to GERD.

Although a hiatal hernia can cause GERD, not all people with a hiatal hernia develop GERD and most people with GERD dont have a hiatal hernia.

Other complications of a hiatal hernia include lung problems or pneumonia, which can occur when stomach contents move up to the lungs.

Is It True That Hiatus Hernias Grow In Size

Heartburn and reflux are usually caused by them. Doctors will treat the hernia with medications if it makes you uncomfortable, according to Dr. Rosen. However, more serious hiatal hernias, also known as paraesophageal hernias, grow in size over time, and the stomach begins to rise further into the chest.

Hiatal Hernia Lifestyle And Medication Treatments

Many people do not experience any symptoms related to their hiatal hernia, or their symptoms, like heartburn, are mild. Generally, the symptoms of hiatal hernias can be controlled with lifestyle changes and medication such as:

    • Eating meals at least three to four hours before lying down

    • Eating moderate to small portions of foods

    • Limiting fatty foods, acidic foods , foods containing caffeine and alcoholic beverages

    • Losing weight or maintaining a healthy weight

    • Over-the-counter antacids or antihistamines your physician may prescribe a stronger medication to manage your symptoms

    • Sleeping in a slightly elevated position

    • Smoking cessation

How Do Doctors Diagnose Inguinal Hernias

To diagnose an inguinal hernia, your doctor will ask about your medical history and symptoms and perform a physical exam. In some cases, doctors also order imaging tests.

During a physical exam, the doctor will examine your abdomen. The doctor may ask you to stand, cough, or strain while he or she checks for a bulge caused by the hernia. The doctor may try to gently massage the contents of the hernia back into the abdomen.

    REPAIR OF HIATUS HERNIA

A hernia is a medical condition in which a fatty tissue or organ pushes or squeezes through a weak spot in surrounding connective tissue or muscle. Hernia generally occurs between our hips and chest. It may also happen at the groin or upper thigh area. The Hernia is not a fatal health condition. But the disease itself is excruciating. It leads to a lot of discomforts. It does not go away on its own. You need to take proper medications or undergo surgery to get rid of hernia.

How Is A Hiatus Hernia Treated

In most cases, symptoms from a hiatus hernia can be kept under control with medicines and lifestyle changes that treat gastro-oesophageal reflux your doctor can help you with these.

Surgery is normally only required if your symptoms become worse and more constant. Rolling, or para-oesophageal, hernias are more likely to require surgery than sliding hiatus hernias.

    Minimally Invasive Hernia Repair

What can I eat after laparoscopic hernia surgery? – Dr. Nanda Rajaneesh

Minimally invasive surgery can effectively diminish the size of a hernia, as well as reduce the opening in the diaphragm, thereby preventing strangulation. During this procedure, surgeons will insert a tiny video camera into your abdomen. Theyll be able to view images projected onto a monitor, which will allow them to complete the procedure with greater control and finesse. The purpose of this surgery is to restore the stomach into your abdomen and close down the hole in the diaphragm. Minimally invasive surgery is associated with a quicker recovery and faster return to function than traditional open repair. Your doctor will determine if you are a candidate for this approach.
